Attach the Lenovo USB-to-DVI Monitor Adapter (45K5296) to the system and set up the original display mode: local LCD primary + monitor adapter secondary.
Then, swap the primary and secondary display between LCD and monitor adapter. The Desktop icons and Toolbar will be separated to different screens.

The same problem can be observed using ATI and NVidia graphics adapters even without the Lenovo USB-to-DVI Monitor Adapter (45K5296) and its driver. It is a windows issue and not due to the Lenovo USB-to-DVI Monitor Adapter.
